During this unprecedented time, we're using new tools and finding new ways to encourage, support and grow young people. 

The following form focuses on consent for online provision. You do not need to grant all permissions but can choose below. If you have any questions about this page, please get in touch via our contact form.

This form is to be completed by a parent or guardian. Please complete the form for each of your young people. 

Young person's details

Do you give permission for your child/dependent to participate online in Youth First provision, including using the third-party platforms and services that we choose?

As we provide new provision and use other third-party platforms, some may require that we get specific consent for them. Zoom is one service that requires this.

Do you give permission for your child/dependent to use and participate in provision via the Zoom service and apps? The young person will be bound by our Privacy Policy and the Terms of Service set by Zoom.

Example: The young person wants to participate in a virtual youth club session for ages 14-17 run on Zoom.

Do you give permission for us to send service updates and marketing messages via Whatsapp to the young person?
Do you give permission for us to send service updates and marketing messages via Whatsapp to you (the parent/guardian)?
Do you give permission for us to use videos, images and other media of your child publically as part of our social media content, website, print materials and general marketing? This includes but is not limited to platforms such as Instagram, TikTok, Facebook, Youtube, Soundcloud and Twitter. Please be aware that this means their image will be available publically and could be shared without our knowledge.

Example: The young person records a video doing the 60-second fitness challenge - we want to publish this video on our social media channels.

Consent for sharing media with other organisations

We may want to share media (photos/videos/sound etc.) of the young person with other organisations for marketing and/or research purposes. Do you give us permission to do this? You do not have to agree to this in order for your child/dependant to be contacted and take part in online youth sessions.

Example: BBC News want to feature Youth First - so we want to send them media of the young person.

Your details

Youth First will process this data in strict accordance with EU General Data Protection Regulation (GDPR). Personal data collected will be stored securely and will be retained for at most 3 years after last contact. For more info please see our Privacy Notice.
